Localizing your ER Passport Form

This help desk article will go through every page of the ER Passport form and show you which elements can be localized and how to localize them. 

What is localization?

In short, "localization" is synonymous with "customization". Every facility starts with a default walk-in template that acts as a basic framework for the majority of our customers. The default template has many "global" elements that are universal across all forms - such as "What is your date of birth?". If you wish to edit these elements to have different wording, you are changing the "global" elements to be "local" to your facility.  This is localizing - customizing universal default elements to be specific for your facility.

Facility Selector Text

This text is completely localizable. 
This text appears on the parent-level Passport welcome page. Users can also access this page by pressing the 'Change location' hyperlink located under the facility's name on the facility-level passport page. This page is only available if there are multiple child facilities with passports turned on.  
Go to Online/Walk-in Module > Localized Settings > Localized ID #38.

Form Instructions

The form instructions are localizable.
These instructions are shown on every page of the form and are a great way to deliver important information or instructions to your users.
Go to Online/Walk-in Module > Localized Settings > Localized ID #41.

Item 1
Mode of Transport options

The mode of transport options can be 'shown' or 'hidden' from users if you decide you do not want to ask this question.
If a user selects any mode of transport other than 'Ambulance', they will be asked to complete an additional Term and Conditions pertaining to emergency transport. Please see the screenshot of 'Page 3' for reference. 
Go to ER Passport Module > Referral Form Settings > Display mode of transport options.

First Time Visit Question

This message is completely localizable. This localization is linked to the online check-in form as well. In other words, any edits you make to this setting will be reflected on the Referral form as well as the Online Check-in form. Go to Online/Walk-in Module > Localized Settings > Localized ID #4.

'What does this mean?' modal
This terms and conditions statement only shows if the patient does not select 'Ambulance' as their mode of transport.
