There are two ways to add clinics and providers: through the ER Express main client site or through the ER Passport form. Typically, facility managers and admins will be adding clinics and providers through the ER Express client site and external clients (such as a referring physician) will be adding providers/clinics through the ER Passport Form. This article will show you how to add clinics and providers with both methods.

Step 1: Navigate to the sidebar and click on ER Passport Module > Referring Clinics & Providers.
Step 2: It is best practice to add the clinic that the provider is associated with before adding the provider. If the clinic is already added, then you can skip step 2 and 3. To add a clinic, click on the blue plus button.
Step 3: Add the referring clinic's name and phone number. Click 'Add clinic'. Click the page-level Save button to save your work.
Step 4: Once you have added the provider's clinic, you are now ready to add the provider. Click on the provider tab.
Step 5: Click the blue plus button.
Step 6: Insert the provider's first name, last name, and credentials in the respective fields. Choose a clinic from the dropdown menu. The clinic's phone number should automatically populate. Add an email to which alerts and updates should be sent for this provider. If this provider works at two different clinics, add another clinic by clicking the 'Add another clinic' button. Click 'Add Provider.' Click the page-level save button to save your work.
How to add Clinics and Providers through the ER Passport form
Step 1: Navigate to your facility's ER Passport form. The URL for this form is unique to all facilities based on their unique facility ID. Here is the URL template, just insert your faciltiy ID where it says [insert facility ID]:

Step 2: Fill out the form as you normally would. Complete the first two pages of the form. The third page of the form is labeled Referral info. This is the page where you will add new clinics/providers.
Step 3: To add a clinic, open up the dropdown menu labeled 'Referring clinic name.' Scroll all the way to the bottom until you see 'Add new.' Click on 'Add new'. This will automatically open up fields to add clinic AND provider information.
Step 4: Add the clinic's name and phone number. Add the provider's first name, last name, and credentials. Add an email to which you want updates and alerts to be sent for the provider. Adding a phone number is optional, but if you do, you must choose which type of phone number it is (work, home, cell). Submit the form and the clinic/provider will automatically be saved to the ER Express application for your facility.
Step 5: If the clinic at which the provider is located already exists, then choose the respective clinic from the dropdown menu. Click on the provider dropdown menu and scroll down to the bottom until you see 'Add new.' Click 'Add new.'
Step 6: Add the provider's first name, last name, and credentials. Add an email to which you want updates and alerts to be sent for the provider. Adding a phone number is optional, but if you do, you must choose which type of phone number it is (work, home, cell). Submit the form and the clinic/provider will automatically be saved to the ER Express application for your facility.
